The names of doctors, hospitals, or clinics where you received treatment.

Any other identifying information that may help locate the records.

For example, telling the VA that you were treated “at a military hospital sometime in the 1980s” is much less helpful than explaining that you received treatment at Womack Army Medical Center while assigned to Fort Bragg between March and June of 1986.

The more specific you can be, the easier it becomes for the VA to locate the records needed to evaluate your claim.

Use VA Form 21-4142 for Private Medical Records

Private medical records often provide valuable evidence because they document ongoing treatment outside the VA healthcare system.

If you’ve received care from private physicians, hospitals, therapists, specialists, chiropractors, physical therapists, or urgent care clinics, the VA may be able to request those records on your behalf using VA Form 21-4142.

Even so, requesting copies yourself is often the faster option. Many private practices close, merge with larger healthcare systems, or destroy older files after certain retention periods. Waiting for multiple record requests to move through several offices can delay your claim. If you already have copies, submitting them directly may help avoid unnecessary delays. If you cannot obtain the records yourself, VA Form 21-4142 gives the VA permission to request them directly from your private healthcare providers as part of its Duty to Assist.

Request Missing Military Records

If you cannot locate your DD214, personnel file, or service treatment records, you may still be able to obtain them through the National Archives and Records Administration (NARA) or other appropriate military records repositories.

Photographs taken during military service, personal letters, emails, journals, deployment orders, unit histories, pharmacy records, insurance claims, civilian medical records created shortly after discharge, employment attendance records, and statements from family members or fellow service members can all help support different parts of a claim.

Rather than sending dozens of unrelated documents, organize your evidence by claimed condition.

For example, if you’re claiming a knee injury, group together treatment records, imaging reports, pharmacy records, and lay statements that relate specifically to that condition.
